Dementia
It is also called as Chronic Organic Brain Syndrome. Dementia affects 5% of those over 65 and 20% of those over 85. General principles of Critical Care Management
Critically ill patients should be assessed regularly and at least twice on morning and evening ward rounds. Initially, it seems daunting to perform such an assessment particularly if there is multiple organ failure and no single unifying diagnosis. A systematic approach is required:
-Receive reports of progress from staff; review specialist opinions and medical/social history.
-Review charts.
-Examination: general and organ specific.
-Assess information from monitors.
-Fluid balance: note previous 24-hour balances, assess intra- and extra vascular state of hydration and set targets for the bedside nurse for the next 24 hours, specifying crystalloid and colloid requirements, the route of administration, and volume and filling pressure limits.
-Nutrition: review calorie intake, route of administration.
-Laboratory results: review haematology including coagulation, biochemistry and bacteriology.
-Review antibiotic therapy: note temperature, white count, line sites etc.
-Review drug chart with ICU pharmacist, consider side-effects and interactions, and identify therapy that can be stopped.
-Review X-rays and other specialist investigations.
-Make an integrated management plan, specifying goals for each organ system.
